Libertas considers communication one of the most important factors in ensuring that home care works for clients, their families, care workers and health professionals alike.
As part of our assurance to create the most effective lines of communication Libertas use 'People Planner'.
What is People Planner and How Does it Help Me?
People Planner is website with a number of different user features.
For our clients and their families it means they can see:
- Who is coming and when
- When a visit is complete
- Invoices (where applicable)
Family members can be given access to the portal (with our client’s permission). So even if family do not live very close, they can keep up to date with client's visits and check progress.
The portal also allows you to update your information, and you can add other people, for example family or a District Nurse.
Associated professionals can also request access to the portal - for more than one client. Community Nurses, GPs, or service commissioners, can access right up-to-the-minute details of the service being provided for clients.
Register for People Planner
On the form you can either request access to the portal or for your visit schedule to be emailed to you each week, or both.
We will check and confirm your application, and send you a link with your password (this may take up to 7 days).
Once you have received access details for the portal (if this was the choice made) you can login through the People Planner button which is on the bottom on every page of our website.
